General practitioners have to deal with an ever-growing group of elderly and chronically ill people in their practice. The burden of care and complexity are increasing and increasingly require expertise and time that are difficult to fit into a busy GP practice. As an experienced geriatric specialist, dr. Nienke can support this in various areas.
Providing CONSULTATIONS for diagnostic dilemmas.
The GP can request (treatment) advice, which can take place by telephone and/or in person. Participation in a multidisciplinary meeting is also possible.
Possible consultation questions:
• questions about medication use, interactions and side effects
• advice on wound and pressure ulcer treatment
• advice on specific geriatric diseases
• advice on treatment of intermittent diseases/comorbidities
• advice and guidance on dementia
• questions about medical decisions in case of incapacity or reduced insight into illness
• Support with advance care/policy discussions
• support with palliative care and end-of-life care
CO-TREATMENT or temporary TAKEOVER OF A TREATMENT following a consultation, if the substantive expertise of the geriatric specialist is desirable for the implementation of the treatment plan.
The duration and division of tasks of the treatment are part of the care treatment plan with care interventions, including consultations and visits, multidisciplinary consultations and adjustment of policy. The geriatric specialist is medically responsible for the specifically agreed treatment. The involvement of fellow (para)medics and/or behavioural experts may also be temporarily indicated in the event of behavioral problems in dementia.
Advice on POLYPHARMACY (medication review) consisting of:
• a pharmacotherapeutic history
• a pharmacotherapeutic analysis
• consultation and coordination with the pharmacist concerned
• a pharmacotherapeutic treatment plan
• 1x per year a follow-up assessment
