What is a Compos Mentis Declaration?
A compos mentis declaration is a medical statement in which a physician assesses whether a person is mentally capable of making their own decisions and understanding the consequences of those decisions at the time of the assessment.
The Latin term compos mentis literally means "of sound mind" or "in possession of one’s mental faculties."
When preparing a compos mentis declaration, the physician evaluates whether the person is able to:
A compos mentis declaration may be requested in situations such as:
A compos mentis declaration reflects the person's mental capacity at the time of the assessment only. It does not guarantee future capacity and does not mean that the person will never experience memory or cognitive problems. Even individuals with early cognitive impairment may still have sufficient decision-making capacity for specific decisions.
The physician typically conducts an interview with the individual and evaluates aspects such as memory, thinking abilities, insight, judgment, and the ability to make and communicate independent decisions. When necessary, relevant medical information may also be reviewed as part of the assessment.
The purpose is to document that a person has sufficient decision-making capacity at the time of the assessment to make a specific decision independently and in an informed manner. This provides clarity and reassurance for the individual, family members, and any legal or professional parties involved.
What Is a Geriatric Assessment?
A geriatric assessment is a comprehensive evaluation of an older adult’s overall health, functioning, and well-being. It looks beyond medical conditions alone and considers how a person is managing in daily life, both physically and mentally.
The goal is to gain a complete understanding of the individual's strengths, challenges, care needs, and opportunities to maintain or improve quality of life and independence.
A geriatric assessment may evaluate:
A geriatric assessment may be helpful for older adults who:
The assessment usually involves one or more conversations with the older adult and, when appropriate, family members or caregivers. The physician may review medical records, medications, and relevant reports, and may perform physical and cognitive evaluations.
Depending on the situation, input from other healthcare professionals such as nurses, therapists, psychologists, or social workers may also be included.
Following the assessment, a report is prepared summarizing the findings and recommendations. These recommendations may relate to:
The purpose of a geriatric assessment is to provide a holistic understanding of an older person’s health and functioning, enabling personalized recommendations that support health, independence, safety, and quality of life.
A delirium is a sudden disturbance in brain function that causes a person to become confused, disoriented, and unable to think clearly. It develops over hours or days and is usually a sign that something is medically wrong and needs prompt attention.
Delirium is especially common in older adults and can be caused by infections, dehydration, medication side effects, pain, surgery, hospitalization, constipation, or other medical conditions.
A person with delirium may:
Symptoms often worsen in the evening or at night.
Although delirium and dementia can both affect memory and thinking, they are very different conditions.
Delirium develops suddenly (hours to days). Develops gradually (months to years). Usually caused by an acute illness or medical problem. Caused by progressive brain disease. Symptoms fluctuate throughout the day. Symptoms are generally more stable from day to day. Attention and alertness are significantly affected. Attention is often preserved in early stages. Often reversible when the cause is treated. Usually not reversible, although treatment may slow progression. Medical emergency requiring prompt evaluation. Chronic condition requiring ongoing care.
Yes. In fact, people with dementia are at a higher risk of developing delirium. When a person with dementia suddenly becomes much more confused, agitated, sleepy, or behaves differently than usual, delirium should always be considered, and medical advice should be sought promptly.
Delirium is often a sign that the body is under stress from an illness or other medical problem. Unlike dementia, delirium is frequently treatable and potentially reversible if the underlying cause is identified and addressed quickly.
Medical attention should be sought as soon as possible if an older adult develops:
A sudden change is often the most important clue that a delirium may be present.
A. DEMENTIA is a GRADUAL decline in BOTH memory and thinking over time.
B. DELIRUIM is a SUDDEN change in mental functioning that develops QUICKLY and often signals an underlying medical problem.

(WHO)
The question remains: how can we achieve this?
- Dementia may affect anyone; how do we respond?
- What is the experience of living with dementia?
- Dementia does not strip individuals of their dignity; rather, it is our reactions and the environment that can do so.
- How can we make society more accessible for individuals with dementia?
- How can people lead safe and responsible lives at home for as long as possible?
Individuals with dementia cannot be held accountable for their condition; their ability to process stimuli diminishes. While learning new information becomes challenging, the emotional needs for social interaction, love, and intimacy remain. As individuals with dementia regress to earlier memories, they may experience anxiety and insecurity due to the increasing dependency on their environment, ultimately requiring 24/7 care.
The healthcare industry recognizes the terms primary and secondary care, which are well understood. Primary care is characterized by its accessibility, allowing individuals to seek care without a referral. Examples include general practitioners, dentists, and physiotherapists. Secondary care requires a referral from a general practitioner and includes specialized medical services provided in hospitals. Recently, a new category has emerged: one-and-a-half-line care, which serves as a bridge between primary and secondary care and highlights the importance of cooperation among practitioners.
As the aging population grows, the number of elderly patients is increasing, leading to a rising demand for care. To maintain affordable healthcare, an intermediate care form has emerged. One-and-a-half-line care involves collaboration between general practitioners and geriatric specialists, with specialists providing treatment advice to general practitioners, thereby facilitating accessible care close to home.
This care model fosters partnerships between general practitioners and medical specialists, promoting knowledge exchange. As the care needs for chronic conditions rise, the primary focus remains on the patient’s interests. One-and-a-half-line care merges the accessibility of primary care with the specialized knowledge of secondary care, enabling more efficient healthcare organization and helping to manage rising healthcare expenditures. Thus, Dr. Nienke can provide effective home healthcare, allowing clients to remain at home as long as possible.
In geriatric medicine, one-and-a-half-line care is already widely used, as general practitioners and other healthcare providers collaborate to coordinate care and enhance service quality. Terms like chain care and network care also describe these collaborative efforts. Dr. Nienke collaborates with medical and paramedical specialists to deliver the best care for her clients.

The Flametree & Hummingbird is actively establishing a private network of professionals in Aruba's 1st-line care sector, aiming to address the complex care needs of the chronic care market. This initiative promotes collaboration among medical and paramedical professionals to provide home-based treatments for clients and their families.

One-and-a-half-line care operates at the intersection of primary and secondary care, emphasizing coordination, collaboration, and knowledge sharing between primary care providers—such as general practitioners—and secondary care providers, like geriatric medicine specialists, to deliver optimal home care services. This partnership is vital in ensuring quality care for elderly patients.

If you need more information about dementia, chronic care management, or supporting a client’s family member, Dr. Nienke can create custom training programs. She frequently presents at conferences, sharing insights on geriatric medicine, palliative care management, and chronic diseases, all crucial aspects of effective zorgmanagement (care management).
